Like so many other risk markers in blood, it is good to have low values and therefore lower risk. It however doesn't show even a tiny bid if plaque is progressing or regressing. Only imaging does. Try to get a maximum carotid intima-media thickness (CIMT) measurement with ultrasound instead. The easiest way to measure plaque growth.
The problem with plaque progressing even despite low risk marker is, that it progresses at least a 30% each year (even on statin and low risk markers). That means with an initial CAC score or lets say 500, a year later is 666, the second year 999, a third year 1333, and so on exponentially, till the next catastrophe. Only imaging shows if it is progressing or regressing
